Blacks die from every chronic disease at a higher rate than any other ethnic group.

For every dollar the median white household has, the median Black household has 10 cents.

WHY WE CARE: Our (Consistent) Vote Matters

Blacks votes when they believe interests are being met, and don’t vote as a protest to inequalities embedded in the political process.

We provideĀ  resources and services to those who need it most.

Community Hero Action Group empowers people in underserved communities in southeastern Pennsylvania by increasing equitable access to resources.Ā  The individuals are typically negatively impacted by social determinants in health SDOH (addressing health disparities, economic instability, and barriers to civic engagement).

The consistent work we do.Ā Ā 

Community Hero Action Group offers consistent, impactful programs to support underserved communities.Ā  Ā Our services include:

  • theĀ  C.A.S.E. Program which empowers seniors with strategies and activation plans to deal with depression,Ā  while the
  • the Her Health Matters Radio Show(96.1 FM) that airs live and focuses on health and wellness topics of African-American women
  • PA Black Health Matters Conference addresses health disparities through education and resources,
  • digital literacy training for seniors to reduce hesitancy in utlizing telehealth services as well as
  • Build the Bloc, a non-partisan voter education and mobilization outreach initiative that takes placeĀ year-round, andĀ 
  • the EXCEL fund which helps increase diversity in the physician and mental health professional health pipelines

We organize events that strengthen our community.Ā  Ā Ā 

We are dedicated to fostering real change through community-driven events.Ā  We create meaningful opportunities for education, connection, and empowerment. Our initiatives address pressing health disparities, promote civic engagement, and support economic stability. Such programming includes:

 

  • PA Black Health and Wealth Conference (formerly known as the PA Black Health Matters Conference)-A yearly conference focused on addressing health disparities and promoting health equity in Black communities
  • Community in Action Festival (formerly known as the Health in Action Festival)-an interactive community event focused on promoting wellness through health screenings, immunizations, connecting community members to health-providers, and family-friendly activities.

When there’s a need, we respond.Ā  Ā Ā 

Community Hero Action Group swiftly responds to emerging community needs by leveraging partnerships to launch targeted, short-term initiatives that address disparities and promote equity which have included:

 

  • Healthy Babies Philly which is an awareness campaign that focuses on vaccine accessibility for infants 0-2 years old

 

 

  • Food Insecurity ā€“A partnership collaboration that combined the importance of providing nutritious food to underserved communities and sourcing from Black farmers.

 

  • Gift of Life Campaign ā€“ Increasing organ donation among African Americans through awareness and education

 

We strengthen communities through civic education and engagement.Ā  Ā  Ā 

Through forums, workshops, and community discussions, we empower individuals with the knowledge and tools to actively participate in civic life. Our civic education fosters informs decision-making and engagement, helping to build stronger, more connected communities, ensuring that every voice contributes to the change we seek.

Our efforts to improve community health and well-being often capture the attention of the media. Through various news features and articles, our impactful work is shared widely, bringing greater awareness to the pressing issues we address and the positive changes we strive to achieve.
